Pre-Visit Tasks
All patients are required to complete the following forms. This can be done in one of three ways:
- By completing the forms online. Click the yellow box below. Select Sarasota for the location.
- By downloading the forms below, completing them, and then hand to our reception on arrival.
- By completing the forms on a clipboard when you arrive. For new patients, ensure you arrive 30 mins before your appointment time.

If you have a signed script from your physician, please ensure that this has been provided to us either in advance or on the day of your visit. If have chosen to self-pay the fee for the liver workup, then no further paperwork is required other than your ID.
On Arrival
When you arrive at our center, please see the front office staff at the main reception in Suite 101. You may need to sign some paperwork and then you can make yourself comfortable in our seated waiting areas.
Technologist Greeting
You will then be greeted by the technologist that will be performing your scan. You will be asked some questions and then directed you to our private changing rooms. For your convenience, we provide lockers to secure any valuables during your test. You may be asked to change into a gown to prevent interference of metal fasteners or snaps in your clothing. Please inform us if you are, or may be pregnant and if you have any medical implants such as a pacemakers etc.
Your technologist will bring you to the scanner and position you correctly on the table and make sure you are as comfortable as possible with some soft pillows. Your scan will then commence. The liver workup generally takes about 45 minutes on the MRI table.
After your scan
Once your scan is complete, your technologist will show you back to the changing rooms so that you can dress again. You are then welcome to leave our center. Your images will be interpreted and the reports will normally be ready the next working day. These will be accessible via the Partners Patient Portal and to your physician of choice through the Partners Physician Portal.
